COVID-19 Visitor Form

To help prevent the spread of COVID-19 in our workplace and community, we’re conducting a screening questionnaire. Your participation is mandatory as a precautionary measure - please complete this form before visiting Canada Post facilities.

All visitors and contractors must wear a face covering that meets Public Health Agency of Canada (PHAC) requirements. 

Please fill out the COVID-19 Visitor Form no more than 24 hours before your scheduled visit to Canada Post facilities.
